About this course
Most maintenance occurrences are closed with a sentence - 'technician error, retraining given' - and the same event returns eighteen months later. This course builds the competence to do the opposite: secure and preserve evidence before it evaporates, interview without contaminating the account, separate active failures from the latent conditions that produced them, and convert findings into corrective and preventive actions that survive an audit. The regulatory spine is Regulation (EU) 2018/1139, Regulation (EU) No 1321/2014 (Part-M, Part-ML, Part-CAMO, Part-CAO and Part-145 management-system and occurrence-reporting rules) and Regulation (EU) No 376/2014 with its reportable-occurrence list in Regulation (EU) 2015/1018. The course works through a published State investigation report end to end, then runs a facilitated desktop investigation, and closes with a server-graded 30-question exam at 80% pass. Written for safety managers, compliance monitoring staff, internal auditors, CAMO engineers, Part-145 quality inspectors, workshop supervisors and lead technicians.
Syllabus
Module 1: M1 - The legal frame and the investigator's mandate
Where the duty to investigate actually comes from: Reg (EU) 2018/1139, the occurrence-reporting machinery of Reg (EU) No 376/2014 with Reg (EU) 2015/1018, the organisational hooks in Reg (EU) No 1321/2014, and the boundary between an internal safety investigation and a State safety investigation.
- Regulatory framework map
- Mandatory and voluntary reporting
- Just culture and its limits
- Mandate scope and independence
Module 2: M2 - Securing the event: evidence, records and accounts
The first four hours decide what the investigation can ever prove. Scene and hardware preservation, the documentary trail through work packs and continuing-airworthiness records, non-contaminating interview technique, and digital or technical data sources.
- First response and evidence preservation
- Documentary and records evidence
- Safety interview technique
- Technical and digital evidence
Module 3: M3 - Analysis: from what happened to why it happened
Structured causal analysis for maintenance and airworthiness events - contributing-factor taxonomies, why-chains and cause-and-effect mapping, barrier analysis, and the separation of active failures from latent organisational conditions.
- Human factors in maintenance events
- Why chains and cause effect mapping
- Barrier and defence analysis
- Active failures and latent conditions
Module 4: M4 - Corrective action and the safety report
Turning findings into corrective and preventive action that changes the system, writing the investigation report an authority inspector and a line technician can both use, and closing the loop through safety feedback and effectiveness review.
- Capa design and effectiveness
- Writing the investigation report
- Safety feedback and lessons learned
Module 5: M5 - Case study, desktop investigation and final exam
A published State investigation report worked end to end, a facilitated desktop investigation you run yourself from evidence to report, and the server-graded 30-question final exam at 80% pass.
- Case study published report
- Desktop investigation workshop
- Final exam event investigator
